I am in agreement about the facial expression of the victim...

but the facial expression of the rapist is what does it for me.

Also, what they're both doing with their hands is important to me. It's always better if the victim's hands are grabbing or clenching something in agony. Say.. bed sheets? Grass?
I also enjoy seeing the penetration point.

There are so many little things that could make or break or a good rape picture.

Clothes on the victim. Pushed up, shoved around, maybe cut or ripped a bit--A bit of clothing left on her adds so much to the drama, and reinforces the understanding that the act is violent and against her will.
