As far as sharing the pics I believe the rule still requires permission from the people actually in the picture. I have grown fuzzy on the details of the rule though, Still better safe than sorry (Even if they did post them elsewhere, they may or may not enjoy being thought of as rape objects to countless faceless strangers who may or may not know them ya know?)

Forcing incest between sisters or female cousins is an interesting idea, more for the psychological and emotional trauma than anything else, IMO. Incest is a fairly strong taboo and forcing a pair of relatives to break that, plus the added stress of forced lesbian intercourse if both are straight, is sure to be very humiliating for the victims. I think it would be entertaining to have a scenario with two women who have an age gap (say, 18 and 25 year old sisters) and use threats against the younger one to manipulate the elder.
